Relationship Series- 2026 Consent Form

Throughout this year, we plan to have a few guest speakers & youth meetings (6-12th grade) on the topic of relationships and sexuality.

Due to the “hot” (no pun intended) nature of this topic, we require a parent consent for students to attend.
